To request any public records, please click the "Public Records Request Form" link below. Although NOT required, the City Clerk's Office respectfully requests that the form be completed to expedite your request and document file activity.
If you do not wish to complete the form, please contact the City Clerk directly at (904) 297-7500, ext. 3307 or email@example.com.
NOTE: E-mail addresses are public records under Florida Law and are not exempt from public records requirements. If you do not want your e-mail address to be subject to being released pursuant to a public records request, do not send electronic mail to this entity. Instead, contact this office by telephone or in writing via the United States Postal Service.
